If you want to send more than one representative for each discipline (PT, OT, Nursing, and optional SLP) you are welcome to do so if you cover their travel expenses.
Participants may extend their stay either before or after the meeting.
The hotel will offer individuals our group rate ($100 per night) for these extra nights.
Please let Julie know if anyone wants extra nights and she will include them on our reservation list we provide to the hotel.

Please note, that I created the example on the attached, and thus, it made need some 'clinical tweaking.' In this layout, the intervention code list is at the bottom of the page.

The portrait version allows the intervention code box to be next to the write-in boxes, but does not allow space for an example.

SLP FORM v4
The PTs and OTs have altered they layout of their forms in order to simplify collection of information. The new format will capture time associated with broader activites and then list the specific interventions used within each. We would like to follow the same format for all our therapy forms, and thus, we have revised the SLP form to match PT and OT layout/concept (now in an Excel file). Your comments are appreciated and and we will discuss on our next SLP call on Tuesday, December 13 at 2:00 ET.
JUST TO REVIEW: We announced on the SLP call and the Clinical Practice Team call that the project will support travel expenses for one SLP per facility to attend the clinician training session in Baltimore if facilities wish to send an SLP (optional). If you don't anticipate that your SLPs will consult with many joint replacement patients I suggest you do not need to attend the training.

Following the call, we solicited advice from several IRB representatives who suggested that facilities that have not yet received IRB approval for the project probably should not fax blinded physician progress notes to the project office. This was suggested on the call so that we could compare information contained on our point-of-care documentation form with information contained in the progress note. Alternatively, we suggest that you complete the form for two patients over three days as suggested and then compare your completed forms to your progress notes.
Please come to the next conference call (December 9 at 11 EST) prepared to discuss similarities and differences between the two.
